Your First Visit
Please call 901-767-9368 for an appointment and you will speak directly to an assistant that will help schedule you to see one of our physicians. She will ask for your insurance information, so please have that information available. She will also ask if you have access to a computer and printer so that you can use the link below to print and complete the following forms before your first visit:
If you do not have access to a computer, the assistant will ask for your address so that we may mail these forms to you. Please bring these forms, all ready completed, with you in order to speed up the registration process. We will also require a copy of your driver’s license and insurance card on this and every subsequent visit.
Your Appointment Day
Please bring your current insurance card and driver's license to each visit. We ask that you arrive 15 minutes prior to your appointment time so that we can update your insurance records or other changes in your personal information. We also want you to know that we verify your insurance benefits and determine your financial obligation in advance. Please come prepared to pay your applicable co-pay and/or deductible at the time of service.
We will do our best to see you at your designated appointment time as we recognize your time is valuable. Due to the nature of our specialty, we will have emergencies that will require the immediate presence of a physician. Babies have a habit of arriving on their own schedule! If this occurs during your appointment time, we will consult with you about your options, which usually are to re-schedule your appointment, see another physician, or wait until your doctor returns. In any case, if you feel that your wait time is excessive please mention it to the receptionist so she can verify the reason for the delay.
Bringing Your Children
Children are naturally a part of our business. We ask that you limit bringing your children to the office to one or two obstetrical visits. We know that you want to include them in this family event. Due to the possibility of transmission of childhood diseases, please never bring your child if you think they may be ill or if they are running a fever. We prefer you do not bring your children to gynecological visits.
Frequently Asked Questions (FAQ's)
Q. When should I schedule my well woman exam?
A. Most insurance companies will pay for a well woman exam once per year, so make sure you schedule your visit on or after the date of your last exam. For your convenience, your nurse will mail you a reminder card with that date.
Q. What if I am running late for my appointment?
A. Please call the main number 901-767-9368 and access the appointment desk! Usually if you are more than 15 minutes late we will need to re- schedule your appointment unless the physician is able to work you in.
Q. Will you remind me of my appointment?
A. As a courtesy to you someone in our Call Center will call to remind you of your appointment the day before. If we get your voice mail or another member of the household, we will leave a message only that we are confirming an appointment and the time. We will not leave detailed appointment information to anyone other than the patient.
Q. Should I bring my records from my previous doctor's office?
A. If you are transferring to our practice while pregnant, or you are transferring with a chronic medical condition, please bring your records with you on your first visit.
